Abstract
We present a software system called eCare for gathering patient information over the web and smart phones from multiple members on the health care team. A technique based on information theory is proposed to measure agreement among team members such as family caregivers and trained professionals. The focus of this effort is on enabling early detection of delirium in community dwelling elderly with dementia based on family caregiver's observations. Based on the data input, the eCare software system uses the Confusion Assessment Method for Family (CAM-FAM) to alert family caregivers and homecare nurses when the elder may be experiencing delirium.
